Men’s pelvic health: Let’s talk about it
Pelvic pain isn't just a women's issue. If you're a man dealing with chronic pelvic pain, discomfort around your prostate, painful urination, or pain during or after ejaculation, you're far from alone. Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) affects approximately 10-16% of men worldwide, that's millions of men suffering with symptoms that are often dismissed, misunderstood, or inadequately treated.
The frustration many men face is real: you've tried antibiotics (even though there's no infection), alpha-blockers that don't help much, and maybe even been told "it's all in your head." Meanwhile, the pain persists, affecting your quality of life, your relationships, your work, and your mental health.
At Pure Balance, we specialize in treating men's pelvic health issues, and we've seen firsthand how acupuncture can provide relief when conventional treatments have fallen short.
What Is CP/CPPS?
Chronic prostatitis/chronic pelvic pain syndrome is exactly what it sounds like: persistent pelvic pain lasting more than three months without a clear infectious cause. Symptoms can include:
Pain or discomfort in the pelvic region, perineum (between scrotum and anus), testicles, or lower back
Painful or difficult urination
Urinary frequency or urgency
Pain during or after ejaculation
Sexual dysfunction or erectile difficulties
Difficulty sitting for long periods
The challenge with CP/CPPS is that conventional medicine often struggles to identify a clear cause or provide effective treatment. Antibiotics work only if there's an infection (which is rare). Pain medications mask symptoms without addressing the underlying problem. And the condition can become chronic and frustrating, with symptoms that come and go or persist for years.
The Research: Acupuncture Works for Men's Pelvic Pain
A landmark randomized controlled trial published in Annals of Internal Medicine in 2021 examined acupuncture's effectiveness for CP/CPPS in 440 men. The results were impressive: compared to sham acupuncture, men who received real acupuncture experienced significant improvements in pain, urinary symptoms, and quality of life and these benefits lasted. At both the 8-week and 32-week follow-ups (six months after treatment ended), the acupuncture group maintained their improvements.
A comprehensive meta-analysis published in 2023 reviewed multiple high-quality studies and confirmed that acupuncture provides measurable benefits for CP/CPPS, particularly in relieving pain. The researchers found that acupuncture outperformed both sham acupuncture and conventional medications, with minimal side effects.
What makes these findings particularly meaningful is that acupuncture appears to work through multiple mechanisms: reducing inflammation, improving blood flow to the pelvic region, modulating pain pathways, and helping release chronically tight pelvic floor muscles. This multi-faceted approach is why acupuncture can succeed where single-mechanism treatments fail.
Why Acupuncture Creates Sustainable Results: The Three-Pillar Approach
At Pure Balance, we treat men's pelvic health through three integrated lenses—what we call the Three Pillars of Pelvic Health. This comprehensive approach is why we're able to create lasting results rather than just temporary symptom relief.
Pillar 1: Biomedicine and Orthopedic Understanding. We assess your entire musculoskeletal system, not just your pelvis. Often, CP/CPPS involves chronically tight pelvic floor muscles, trigger points in surrounding muscles (hip flexors, glutes, low back), and compensatory patterns from old injuries or repetitive activities. Using acupuncture and dry needling, we release these tight muscles and restore proper function. If you sit for long hours at work, have a history of heavy lifting, or play sports that stress the pelvic region, these patterns matter—and we address them.
Pillar 2: Body Systems and Traditional Chinese Medicine. In Chinese medicine, pelvic health in men is primarily governed by the Kidneys and Liver. Kidney deficiency can manifest as chronic pelvic pain, urinary issues, and sexual dysfunction. Liver Qi stagnation creates pain, tension, and inflammation in the pelvis. By treating the body as an integrated system and addressing these underlying patterns, we help restore balance and function. This is why acupuncture often improves multiple symptoms simultaneously, when we restore the free flow of Qi and Blood through the pelvis and strengthen the underlying systems, pain decreases, urinary function improves, and sexual health often rebounds.
Pillar 3: Emotional and Spiritual Support. This might be the most overlooked aspect of men's pelvic health, but it's crucial. Chronic pelvic pain is intimately connected to stress, anxiety, and the nervous system. Many men with CP/CPPS have nervous systems stuck in fight-or-flight mode, creating chronic muscle tension and pain. Additionally, there can be significant psychological distress, anxiety about symptoms, fear around sexual function, and frustration with lack of improvement. Acupuncture works directly with the nervous system to shift you out of sympathetic dominance into a more relaxed state. This isn't just about feeling calmer—it's about creating the physiological conditions where your pelvic floor can actually release and heal.
Our Approach at Pure Balance
When you come to Pure Balance for men's pelvic health concerns, we take a thorough history and create an individualized treatment plan. We use acupuncture and dry needling to release tight pelvic floor muscles and trigger points, address larger muscle imbalances throughout your body, improve circulation to the pelvic region, reduce inflammation and modulate pain pathways, and support nervous system regulation to reduce stress and chronic tension.
Because we're a cash-based practice, we have time for comprehensive assessment and treatment, not a rushed 15-minute appointment with a generic protocol.
3 Things You Can Do at Home
Practice Deep Breathing Daily. Your pelvic floor and diaphragm work together. Chronic shallow breathing creates pelvic tension. Spend 5-10 minutes daily practicing slow, deep belly breathing. This activates your parasympathetic nervous system and helps your pelvic floor relax—essential for healing.
Address Your Sitting Posture. If you sit for work, take breaks every hour to stand and walk. Consider a standing desk for part of your day. When sitting, use a cushion that reduces pressure on the perineum. Chronic sitting, especially with poor posture, contributes significantly to pelvic floor dysfunction.
Reduce Inflammatory Foods and Manage Stress. Inflammation worsens pelvic pain. Minimize sugar, processed foods, and excessive alcohol. Include anti-inflammatory foods like fatty fish, vegetables, and healthy fats. And find stress management practices that work for you—whether that's exercise, meditation, time in nature, or therapy. Chronic stress perpetuates chronic pain.
You Don't Have to Live with This
CP/CPPS can feel isolating and frustrating, especially when conventional treatments haven't helped. But you don't have to accept chronic pelvic pain as your new normal. The research is clear: acupuncture provides effective, lasting relief for men with pelvic pain.
If you're ready to try a comprehensive, evidence-based approach that addresses the biomechanical, systemic, and emotional-spiritual aspects of your condition, we invite you to schedule a consultation at Pure Balance. Let's work together to help you reclaim comfort, function, and quality of life.
